Health and Personal Care

When new members join River House, an individualized Care Plan is designed in collaboration with the member, their family, and their doctor. Our staff of RNs and health aides oversee the implementation of the plan by providing:

  • Individual assessments
  • Health monitoring
  • Administration of physician-prescribed medications and treatments
  • Coordinated care with personal physicians
  • Assistance with personal hygiene
  • Toileting

As members become comfortable at River House, additional services that may be difficult for family members to assist with, such as bathing, haircuts, and shaves, can be provided at an additional cost.

About Individualized Care

The care we provide to our clients is person-centered, meaning we are respectful of, and responsive to, their preferences, needs, and values. We encourage our members' independence and autonomy by having them partake in the development of their Care Plans, and ensure that their voices are heard. Our members are able to choose what activities they participate in, to guarantee that they are having meaningful and joyful experiences here at River House.

About Our Care Plans

Each client has a written Care Plan completed within thirty days of admission. Care Plans consist of, but are not limited to, an analysis of the member's strengths and needs; comprehensive medical diagnosis; mental abilities or disabilities; socialization level; therapy recommendations; as well as goals and objectives for health and recreation. These plans ensure that our members are receiving optimal care based on their unique needs. They can be adjusted at any time with input from caregivers, doctors, and River House nursing staff.
